Year 3: do the selective that is part of the Surgery-Anesthesia Clerkship or seek out other electives that can expose you to the field. Meet with Dr. Colvin mid-year to start to discuss the application process and plan for electives in the summer and early fall of year 4.
Year 4: sub-internship, away rotations

Yes; away electives are an essential component of the application process and most applicants do 2-3 away electives in addition to an orthopedics subinternship at their home school.
