Human and System Contributors to errors in Health Care

PATIENT SAFETY: LATENT RISK FACTORS

Session Description: Adverse events and untoward outcomes in healthcare are almost always the result of a cascade of errors rather than a single error. Since the factors that promote errors are not directly visible in the working environment, they are described as latent risk factors. In this session, we will understand how these latent factors affect safety and can enable us to design more effective control measures that will impact both individual performance and patient outcomes.
