POPULATION HEALTH
Session Description: Population health is defined as the health outcomes of a group of individuals, including the distribution of such outcomes within the group. The inherent value of a population health perspective is that it facilitates integration of knowledge across the many factors that influence health and health outcomes. This session introduces students to this perspective.

Learning Objectives: At the end of this session, the student learners will:
- Define the multiple determinants of health (Kindig Model)
- Recognize how populations are defined and how this concept relates to the Affordable Care Act and Accountable Care Organizations
- Discuss panel management and how EHRs are a tool to help physicians engage in population health
- Cite successful examples, both locally and nationally, of population health strategies
