CLINICAL INFORMATION FOR NOTES AND PRESENTATIONS
Aortic stenosis: last echo, history of syncope, dyspnea, angina
Asthma: steroid requirement, other drugs/dosages, ED visits, hospital and ICU admissions, history of intubation, peak flow measurements

Atrial fibrillation: paroxysmal vs. persistent vs. permanent.
- If acute: inciting factors (drugs, acute cardiac/pulm/endocrine issues)
- If chronic: drugs/dosages. Warfarin use: most recent PT/INR. History of CVA/TIA
CHF: drugs/dosages, NYHA class, physical exam results (ankle edema, crackles, JVD, hepatomegaly), exercise tolerance, dyspnea, orthopnea. Last echo (valve disease, cardiomyopathy), history of MI, CVA, DM, obesity, smoking, BP and lipid levels.
Dementia: MMSE results, duration, EtOH/B12 status, head imaging results, depression screening results
DM: A1C, drugs/dosages, retinopathy, neuropathy, nephropathy
Hypertension: BP range, drugs/dosages, history of MI, CVA, CHF, kidney disease, hypertensive crisis, endocrine disease, smoking
Hypercholesterolemia: drugs/dosages, most recent lipid panel, history of MI, CVA, renal disease, DM, AAA, smoking, activity level
MI/CAD (Past): Drugs/dosages, BP range, cholesterol levels, History of CABG/PCI, angina
Osteoarthritis: drugs/dosages, number of joints, signs of inflammation (swelling, warmth), trauma history, imaging results
Osteoporosis: Drugs/dosages, fracture history, steroid use, DEXA scan results, history of endocrine disorders and renal disease
